JOB DESCRIPTION FOR CAMPO URBANO MENTOR/JOB COACH
Duties will include job coaching people with, in some cases, severe and multiple disabilities; may include facilitating meetings, including personal futures planning, to build natural supports, community involvement and employment for people with disabilities.
NATURE AND SCOPE OF POSITION
The position requires working with a small number of individuals who live in the local community. Travel to community job sites may be required.
The position reports to the Program Coordinator and/or Ranch Manager.
The Blake Foundation is a nonprofit organization serving adults with developmental and/or physical disabilities.
The Blake Foundation operates year round on a weekly schedule. The full-time Job Coach will work a 40-hour week. Exact days and hours may vary with the demands of community jobs. Occasional evening and weekend hours may be required.
Assist Ranch manager in assigning tasks to participants.
Help design tasks and training strategies to promote a productive work environment. Tasks will include caring for ranch livestock, maintaining plants and garden, and upkeep of the Ranch buildings along with promoting sales of natural products developed at the Ranch.
Encourage social integration for employees. Provide training, formal support and opportunities for interaction in the community.
Keep individuals’ file up-to-date, including weekly activity logs and monthly summary reports, collecting ongoing data on skill implementation, generate monthly reports, document professional and family contacts initiate and file incident reports.
Assist employees with personal-care needs (eating, using the restroom, transferring) as required.
Attend in service training as deemed necessary by supervisors.
Perform other related tasks as assigned.
Conform to policies, practices, and procedures of Easter Seals Blake Foundation.
REQUIRED SKILLS, KNOWLEDGE AND ABILITIES.
Ability to work independently, without daily supervision, in carrying out many of the responsibilities of the position.
Ability to effectively communicate both orally and in writing.
Demonstrated knowledge of training and supervision procedures.
Knowledge of the individual planning process, strategies for teaching job tasks methods of monitoring progress.
Applicants should have a minimum of a high school degree with some work experience in job coaching or day program preferable. Any combination of education and experience which provides the required skills, knowledge, and ability will be considered.
Ability to use work time efficiently, to arrive at work and scheduled appointments on time, not to abuse sick time or telephones.
